Outpatient Clinic Services


The agency provides a wide array of adult and child services through the Outpatient Clinic. These services are designed to meet the needs of individuals who are experiencing psychiatric, emotional, and behavioral issues. Qualified staff provides all services, in a timely and confidential manner. Services are designed based on the need of the individual and help to promote stability, growth, and recovery. Services are provided within the clinic, home, or community setting. Specific services include the following...

  • Diagnostic Assessment, individual service planning, case management and referral
  • Psychiatric assessments, medication administration, medication monitoring, nursing services
  • Counseling and therapy (individual and group)
  • Screening, assessment, and support services for children/adolescents
  • Assistance in acquiring food, shelter, clothing, medical assistance, advocacy, and linkage
  • Post hospital discharge follow-up with linkage and referral to all support services needed to assist the individual in achieving and maintaining stability while living in the community
  • Psychosocial Rehabilitation for individuals with severe and persistent mental illness
  • Crisis Intervention available 24 hours a day, 365 days a year during regular business hours and after hours
  • Services can be provided at Pana Community Hospital by appointment
